In analyses of the largest Macedonian companies, the focus is usually on revenue, profit and position in rankings. But at Alkaloid, there is another indicator that explains its business strategy much better – the sales structure.
While many domestic companies are tying their future to the Macedonian market, Alkaloid has been building growth through internationalization for a long time. In 2024, 68% of consolidated sales were realized on foreign markets, while the domestic market accounted for 32%.
This ratio shows that the company does not depend exclusively on domestic demand. On the contrary, the majority of its revenues are generated through competition in foreign markets, where the pharmaceutical industry operates under significantly stricter regulatory requirements and stronger international competition.

For an industrial company, especially in the pharmaceutical sector, exporting is more than just selling abroad. It involves meeting international manufacturing standards, registering products in different markets, building distribution networks, and long-term partnerships with healthcare systems and pharmaceutical companies.
In this context, the fact that Alkaloid exports to more than 50 countries is an indicator of the breadth of its international presence. The company today has 26 subsidiaries and two representative offices outside the country, and in 2024 it expanded its presence with a new company in Croatia and a capital-affiliated company in Kazakhstan.
Such a strategy allows for greater diversification of risk. Instead of results being dependent on economic movements in a single market, revenues are distributed across multiple regions, which creates greater resilience in the face of economic and geopolitical uncertainties.

Behind this export expansion is a continuous investment cycle. According to the annual report, in the past 15 years Alkaloid has invested more than 230 million euros in production facilities, laboratories, research and development, modern technology, digital systems and employee education.
In the pharmaceutical industry, such investments rarely bring immediate financial impact. A new manufacturing facility, laboratory, or development platform may take years to contribute to revenue, but it is these investments that determine a company's competitiveness in the long term.
Therefore, investments in this sector are not just a capital expense, but a foundation for future growth.
In addition to financial results, 2024 was also significant in terms of the expansion of corporate infrastructure.
With the establishment of ALKALOID PHARMA INTERNATIONAL DOO Zagreb , Alkaloid gained its eighth company in a member state of the European Union. At the same time, with the establishment of Alkaloid Kazakhstan LLC , the company took the first step towards a direct presence in the markets of Central Asia.
These moves indicate that international expansion is not based solely on exporting through distributors, but also on gradually building its own presence in selected markets.

Within the framework of the 200 Largest and Most Successful project, Alkaloid regularly ranks among the leading companies in terms of revenue and profit. But an analysis of its business model shows that its real competitive advantage lies in something else – the ability to create products that are successfully marketed outside the domestic market.
For the Macedonian economy, such companies have a broader significance than their financial results. They generate foreign exchange inflows, create highly qualified jobs, invest in technology and knowledge, and increase the visibility of domestic industry on international markets.
At a time when competitiveness is increasingly measured by the ability to succeed in the global market, the example of Alkaloid shows that long-term growth is not built solely through increasing sales, but through continuous investments, expanding international presence, and creating products that can meet the highest standards in the pharmaceutical industry.
